In this section of the handbook, we aim to empower transgender individuals with a diverse array of sources where they can ask questions, seek guidance, and find the support they need.

Organizations and How to Reach Them

Trans United Europe

Trans United Europe focuses on policy advice

regarding Black and People of Color (BPOC) trans issues. They engage in activism for legal changes, advocate for inclusion in anti-racism debates, organize the Transgender Day of Remembrance (TDOR) in the Netherlands, and promote trans sex work activism and outreach. They provide safe spaces and collaborate with social workers, lawyers, police, and mental health care professionals. Additionally, Trans United Europe has established a trans-led health clinic for trans sex workers, with a special focus on non-binary and trans men. In the Netherlands, the usual trajectory for trans individuals seeking to affirm their gender identity starts with psychiatric counseling and a gender dysphoria diagnosis. This step involves working with a mental health professional who specializes in gender issues. Through a series of sessions, the counsellor assesses and supports individuals in exploring their gender identity, providing a safe space for self-reflection and understanding.


Unfortunately, mental health care (GGZ) and transgender health care in the Netherlands are struggling with capacity and budgeting issues. So, beware that it’s a long process: waiting lists can vary from 1 to 4 years.

Endocrinology

After receiving a gender dysphoria diagnosis, individuals may be referred to an endocrinologist. Endocrinology plays a crucial role in the medical transition process for many trans individuals. With the guidance of an endocrinologist, hormone replacement therapy (HRT) can be initiated. HRT involves the administration of hormone medications, such as testosterone for transmasculine individuals or estrogen and anti-androgens for transfeminine individuals, to help align their physical characteristics with their affirmed gender identity. Regular check-ups and monitoring of hormone levels are essential to ensure the well-being and safety of individuals undergoing HRT.

Endocrinology
Seeking Support and Resources for DIY Hormones

In some cases, the long waiting lists for gender-affirming care can lead individuals to consider self-administering hormone replacement therapy (HRT) on their own. While this is not recommended, we understand the reasons behind such decisions. It is important to note that starting HRT independently does not impact your future treatment by a gender team. Therefore, there's no need to keep it a secret when your turn for official care comes. However, it is crucial to inform your general practitioner (GP) about your DIY hormone usage. 
 

Additionally, seeking guidance and support from experienced individuals who have gone through this process can be immensely helpful. There are support groups available in Amsterdam and The Hague that specifically focus on DIY hormones. 
 

Furthermore, organizations like the Trans Radical Resource Exchange (@trrex_denhaag) in The Hague or online communities such as the "TransDIY Discord" can provide valuable assistance. These sources offer a platform to connect with others and access knowledge about hormone therapy, medication, blood testing, and safe binding. Remember, you don't have to navigate this journey alone—reaching out for support and guidance is crucial for your well-being and safety.

Seeking Support and Resources for DIY Hormones
Surgery

For some individuals, gender-affirming surgeries are an important step in their transition. In the Netherlands, access to gender-affirming surgeries is facilitated through specialized gender clinics. These clinics collaborate with multidisciplinary teams, including surgeons, psychologists, and endocrinologists, to provide comprehensive care. The surgical procedures offered may include chest reconstruction (top surgery), genital reconstruction (bottom surgery), and various other procedures tailored to individual needs. The decision to pursue surgery is a personal one, and it is important for individuals to receive adequate information, support, and guidance throughout the process.

Recognizing the diversity of gender identities and experiences, the Netherlands also offers transitioning care for nonbinary individuals or those who seek unorthodox paths. The approach for nonbinary individuals is highly individualized, focusing on understanding their unique needs and goals. The care may involve hormone therapy, social transition support, and counseling tailored to their specific situation. Gender clinics and healthcare professionals work closely with nonbinary individuals to provide affirming and respectful care, supporting their journey towards self-discovery and self-expression.

It is important to note that the trajectories described here are general outlines, and the experiences and preferences of individuals may vary. The healthcare system in the Netherlands is committed to providing comprehensive and individualized care, ensuring that trans individuals have access to the support, resources, and medical interventions they need to live authentically and comfortably in their affirmed gender identities. For access to any (trans) healthcare the ‘restitutiepolis’ is recommended. The monthly costs are a bit higher but it will make sure you can access any (trans) healthcare without having to contribute financially yourself later on.

If you have a low income, it’s worth checking if you’re eligible for the ‘zorgtoeslag’ by the state.

Transfer of Medical Files and Validity of Previous Diagnoses in the Netherlands

Transferring medical files and the validity of previous diagnoses made abroad is an important consideration for individuals who have started their transition journey outside of the Netherlands. While the process can vary depending on individual circumstances, the Dutch healthcare system generally recognizes and respects previous diagnoses and treatments.


If someone has already been diagnosed with gender dysphoria and initiated hormone replacement therapy (HRT) in another country, they may be able to continue their transition journey in the Netherlands without starting from scratch. However, it is crucial to have their medical records and documentation transferred to Dutch healthcare providers.


To initiate this process, individuals should consult with a Dutch general practitioner (GP) who specializes in transgender healthcare. They can provide guidance on the necessary steps to transfer medical files and ensure the continuity of care. The GP may request relevant documentation, such as medical records, diagnostic reports, and treatment history, to assess the individual's needs and determine the appropriate course of action.


In some cases, the GP may prescribe HRT based on the prescription from the individual's previous healthcare provider abroad. This prescription may need to be reviewed and validated by the GP, taking into account local guidelines and regulations. It is important to note that the availability and coverage of HRT medications may vary, and individuals may need to consult with their healthcare provider and insurance company to determine the specific details regarding costs and coverage.


In instances where individuals encounter challenges in obtaining their prescribed medication from a Dutch pharmacy, seeking alternative options, such as pharmacies that may be more accommodating, can be beneficial. For example, in the case mentioned, the pharmacy in Plein was willing to provide the prescribed Testosterone, albeit with out-of-pocket payment. However, with a prescription from a Dutch GP, it may be possible for the medication to be covered by insurance.


Navigating the transfer of medical files and the continuation of a transition journey in the Netherlands requires collaboration between the individual, Dutch healthcare providers, and relevant stakeholders. By working closely with healthcare professionals who specialize in transgender care, individuals can ensure a smoother transition process, build upon previous diagnoses and treatments, and receive the appropriate support and medical interventions within the Dutch healthcare system.
